It is a simple list. Keep ready your name and father's or husband's name as in records, your village, block and district, your mobile number, and your land details, that is the survey, khasra or khata number and the area of the plot. Aadhaar is commonly asked to link the record, and a copy of your land record or land receipt helps confirm the plot. You will also tell them which crop or crops you plan to grow on that field, because the card prints a crop-wise recommendation. If you are submitting your own sample rather than going through the sampling drive, bring the correctly collected and dried sample in a clean cloth bag with a label showing all these details. Giving a correct, active mobile number matters because the status and ready notification often come by SMS. Confirm the exact document list with your block agriculture office or KVK, as it can vary slightly by state.
Take your khasra number and Aadhaar and a working mobile number. They linked my card to the plot with the khasra and I got an SMS when it was ready.
